I acknowledge that I am requesting Caddy Service from Caddy Direct and I understand that any caddy provided operates as an Independent Contractor and is not an employee of Caddy Direct or New Jersey National Golf Club. Requesting a caddy and acknowledging I have read this release by checking the box, indicates the complete and unconditional release of any and all liability of Caddy Direct and New Jersey National Golf Club. I assign against and from any and all losses, liabilities, damages, and expenses from any suits brought against Caddy Direct and/ or New Jersey National Golf Club. I agree to this release and its enforcement to the greatest extent, by the laws of the State of New Jersey.
